There are two main ways to obtain the upgrade:

1. Purchase from Bonny Sims that can be found in Skyhold. The price is 1291 Sovereigns
2. Looted from one of the chests during the quest: What Pride Had Wrought. Look for it in one of the Red Templar camps.
