Obtained during Trials of the Crows quest. You will be tasked to go to Kadan-Fe Hideout in the World Map. This is where the item can be obtained from a chest.

How to obtain the Trials of the Crows quest? You will have to finish the main story quest and encounter Zevran. Then go to the Denerim Market District and talk with Master Ignacio

After that, try to leave the area through the main gate. A messenger will approach. After that go to Gnawed Noble Tavern where you can meet Ignacio in one of the rooms and start the Trials of the Crows.
