The helmet can be obtained during the Redcliffe Village main quest. You have to have no casualties during the quest: The Attack at Nightfall

If you are successful Bann Teagan will reward you with this helmet

Helm of the Red - Notes

The helmet can be obtained fairly in the game. I recommend using it on offensive warrior build.
