There are multiple ways to get the helmet, however, the guaranteed one is this:

• Cesar's Special stock - you have to complete The Trial of the Crows to unlock it

Here are other ways to obtain it:

• Human Noble Origin quest - random chance to drop from Arl Howe's Knight
• Can drop randomly from Lieutenant Mercenary during the quest Last Request quest
• Can randomly appear in Redcliffe Castle vault in one of the chests
• Marjorlaine's Home - during the Leliana's Past side quest.

• To upgrade the helmet's tier you can put it in Party Storage and leave the area for a while.
• Another option for upgrading: you can also sell it to Mikhael Dryden and come back to the area and buy it back.
• The helmet is best suited for builds that use active skills and need large amounts of stamina
