Juggernaut Helm - How to obtain

West Brecilian Forest, Eastern part of the area. Look for a tombstone and interact with it. At first, there may not be an option to "Disturb the Wards", so check other tombstones. Once you are able to do that, enemies will spawn and you can get the helm from them.
