Purchased from Lann Tarv in Moonrise Towers in Act 2. The item requires convincing Disciple Z'rell that you need additional aid to retrieve Kethric's Relic. After this Lann Tav will have additional items to sell.
Be sure to enter the building peacefully as his merchandise will not be available otherwise.

FYI, you need to speak to Z'rell with an insight dialogue "when you ask about why it's so important." There you'll get more support, and Bugbear will update his store's inventory.
Can also be obtained after defeating and looting Lann Tarv during the attack on moonrise tower additional to purchasing it previously. So the item can be acquired twice. (In my case as least)
