Located at Lora's House. Received as a reward for the quest 
Save Vanra.
To save Vanra, you either have to knock out The Hag (Lv. 11) and cut it out of her or use 
Hag's Bane. To get the recipe for it you need to finish Help the Hag Survivors quest.
