Moonrise Towers First Floor. You will have to gain access to Balthazaar's secret room. On the Balthazar's Bookshelf interact with"Bookshelf Labelled 'Musty Tomes'" and an ancient altar will appear. You can insert a heart that a dead Balthazaar's Apprentice carries nearby.
The hat will be inside a locked Opulent Chest.
